TCARE Assessment
We ask that every caregiver first fill out the TCARE Caregiver Assessment form, which seeks to identify a person’s level of stress and risk of caregiver burnout. Based on the results, they can be connected with a Family Caregiver Support Program (FCSP) social worker to create a tailored plan to support their caregiving journey.
The FCSP offers caregiver training, support groups, home adaptations and safety devices, a caregiver resource library and computer center, information and assistance, and community education for eligible caregivers. It may also help caregivers with in-home respite and out-of-home respite (day care) for caregivers who are at risk of burnout.
Family Caregiver Resource Centers
The VCAAA and two other organizations have divided up Ventura County and created Family Caregiver Resource Centers (FCRC). Please choose from the list below to find the program site in your community.

Preocupaciones de los mayores – Headquartered in Thousand Oaks and serves east Ventura County (i.e., Thousand Oaks, Simi Valley, Moorpark, the portion of Westlake Village in Ventura County, and unincorporated areas of east Ventura County including Newbury Park). VCAAA – Headquartered in the Santa Clara Valley, “La Buena Vida” serves all caregivers in west Ventura County (including Santa Paula, Fillmore, Piru, Ojai, Ventura and Oxnard). In-Home Supportive Services (IHSS)
Administered by the Human Services Agency, IHSS helps qualified older adults and people with disabilities receive a wide variety of in-home support services to remain safe and comfortable in their home.
IHSS providers can help with domestic and related tasks, personal care and medical accompaniment. A provider may be a friend or a family member.
To be eligible for IHSS, you must live in the United States and be a California resident. If you’re not already receiving Medi-Cal, you will need to apply for it. You must also live in your own home or a dwelling of your choice.

Trualta
Trualta, an online tool for managing care at home, is available for free as a result of a partnership with the VCAAA.
Trualta delivers engaging and personalized training; short, quick lessons created by experts in aging; professional-level training adapted for informal caregivers; and 24/7 accessibility.
No app is required. The site can be viewed on any browser, computer, laptop, phone or tablet.
Family Caregiver Preplacement Counseling
